First Aid New
Vanilla Minecraft gives you one health bar and calls it a day. First Aid New throws that idea out the window and replaces it with a full locational damage system, where every arrow, axe swing, and creeper blast lands somewhere specific on your body. This is a multi-loader port of ichttt's classic First Aid mod, rebuilt from the ground up for modern Minecraft, and it turns survival into something that actually feels physical.
What Makes First Aid New Different
Instead of a single pool of hearts, your character has separate health values for the head, body, arms, legs, and feet. Damage applies to the part that got hit, and each region has its own overflow rules. Take a sword to the leg and you will feel it in how you move. Catch a hit to the arm and your mining and fighting suffer. The mod does not just track numbers; it translates them into consequences.
Locational Health and Injury Debuffs
Every limb carries its own pool, and injuries stack debuffs that change how you play. A wrecked leg slows you down. A battered arm weakens your swings. You cannot simply chug a golden apple and pretend nothing happened, because the damage is tied to where it landed.
Medicine With Actual Pacing
Healing is not instant. Bandages, plasters, painkillers, morphine, and injectors all have activation delays and heal-over-time effects. The Morphine Injector now costs two Morphine plus iron, redstone, and a glass bottle, so concentrated relief is a real investment rather than a one-pill craft. Milk fully clears morphine model state, which means no sticky 00:00 timer lingering on your screen.
Unconsciousness, Rescue, and Suppression
Critical damage can knock you out completely. You enter a downed state with a give-up flow and a revive window, so a friend with a bandage can pull you back from the brink. Meanwhile, projectiles passing near you trigger suppression: desaturation, blur, vignette, and optional tinnitus that make firefights feel genuinely tense.
Client Feedback and the Nervous System
The 1.3.0 release treats the HUD like a nervous system. Pain is no longer a flat filter, and morphine is no longer a binary wash. Morphine saturation fades with remaining duration, so the last minutes of calm look different from the first. Stronger suppression desaturates harder, adds pain-like blur under fire, and applies a continuous gray-white edge wash that scales with pressure. The adrenaline injector locks screen blur to moderate pain strength, giving you tunnel vision for the fight even while painkillers are active. Add heartbeat audio, hit red pulses, and a morphine color grade, and every dose writes itself onto the glass of your screen.
There is also an opioid addiction system with a hidden addiction value, withdrawal episodes, and status icons. Addiction still rises on use, but the rise and withdrawal icons no longer sit on top of an active morphine dose. Third-party mods can hook into a public extension API to register custom treatment items and direct-use medicines, so modpack authors have room to build on top of the framework.
Supported Versions and Loaders
First Aid New ships as separate artifacts for each loader and Minecraft version. The current builds cover Forge 1.20.1, Fabric 1.21.1, NeoForge 1.21.1, Fabric 26.2, and NeoForge 26.2. If you run a 1.20.1 Forge pack, grab the matching Forge file. If you are on a newer Fabric or NeoForge setup, the 26.2 artifacts are the ones you want. Mixing loaders will not work, so double-check your mod loader before you download First Aid New.
How to Install First Aid New
- Install the correct mod loader for your Minecraft version (Forge, Fabric, or NeoForge).
- Place the matching First Aid New jar file into your mods folder.
- Launch the game once to generate config files, then close it.
- Edit the config or use in-game commands to tune pain, suppression, medicine timing, and injury debuffs.
- Restart and test with /damagePart HEAD 4 to confirm locational damage is working.

Commands and Presets
Operators get a compact command tip on join, with clickable bracketed commands that prefill chat. The main toggles cover pain feedback, suppression strength, medicine pacing, revive wake-up delay, and injury debuffs. A survival-oriented preset runs dynamic pain, dynamic suppression, realistic medicine, a 15-second revive wake-up, and normal injury debuffs. A balanced preset softens suppression to mild and drops injury debuffs to low. A casual preset uses mild pain, mild suppression, casual medicine, no revive wake-up, and low debuffs. Admins can also set addiction values directly for balance testing.
First Aid New is not trying to make Minecraft harder for the sake of it. It is trying to make damage mean something. When a hit to the leg changes how you retreat, when morphine fades visibly as it runs out, and when a downed teammate needs a real rescue window, survival stops being a number and starts being a story. That is the deal this mod offers, and it is worth taking.
